    Summary

    The Clinical Pharmacy Acute Care / Community Living Center Supervisor is a licensed pharmacist with a Doctor of Pharmacy degree who has completed an American Society of Health Systems Pharmacists accredited Clinical Pharmacy Practice Residency, or other equivalent experience.

    He / she is responsible for the daily professional and administrative / personnel management of the assigned clinical pharmacy program staff.

    Qualifications Preferred Experience : In addition to basic Qualification Standard, completion of ASHP accredited PGY-1 Pharmacy Practice Residency and / or PGY-2 Specialty Pharmacy Residency (in an applicable specialty) or equivalent experience in the major duties for at least 3 years is preferred.

    Applicants pending the completion of educational or certification / licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

    Basic Requirements :

    • Education Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and / or a Doctor of Pharmacy (Pharm.D.) degree
    • OR Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT)

    • Such individuals have responsibilities for supervising multiple pharmacists and technicians
    • Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals
    • Develops performance standards, assures that the program area is compliant with all regulatory and accrediting body requirements, designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities
    • Develops policies and procedures relative to their assigned area
    • Pharmacists assigned to this position must demonstrate the following KSAs : 1
    • Ability to effectively supervise subordinate staff
    • Ability to communicate orally and in writing to persuade and influence clinical and management decisions
    • Expert understanding of regulatory and quality standards pertaining to pharmacy
    • Skill in managing people or programs

    Responsibilities The functions of the position include, but may not be limited to the following : Administrative / Supervisory Coordinates the aspects of the pharmacy related activities within the assigned clinical pharmacy program area to develop, organize, and manage these complex program areas.

    Promotes safe and rational drug therapy to the assigned clinical pharmacy program patients by providing comprehensive medication management services Maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals.

    Ensures implementation of performance standards, assures that clinical practice is compliant with all regulatory and accrediting body requirements.

    Develops policies and procedures relative to the assigned area of practice Interviews prospective candidates for appointments, recommends appointments, advancements, or promotions or disciplinary actions.

    Clinical Acts as a provider, with specialty training in Acute Care and CLC with a scope of practice, providing expertise to other providers, pharmacists, nursing and other disciplines, as appropriate.

    • As part of the interdisciplinary team, appropriately renders care to acute care and CLC patients, including, but not limited to, prescribing medications, to include initiation, continuation, discontinuation, and altering therapy under the Clinical Pharmacy Specialist's Scope of Practice;
    • physical assessment; monitoring medication regimens; ordering and analyzing necessary laboratory tests; adjusting medications;

    and assisting in the management of medical emergencies, adverse drug reactions, and acute and chronic disease states.

    Provides oral and written consultation with other health care professionals regarding drug therapy selection or management (including drug induced problems) in an accurate, timely and professional manner.

    Designs a therapeutic regimen or plan, based on patient-specific information that includes establishing a therapeutic endpoint, electing an appropriate drug and related therapy, and establishing an appropriate dosage regimen.

    From a verbal or written patient presentation of signs, symptoms, laboratory tests and diagnoses, recognizes and lists important medical problems, disease states, symptoms or abnormal laboratory values that may necessitate altering the therapeutic regimen or which may be induced by drug therapy.

    • All patients' interviews, assessments and interventions are documented appropriately in a manner consistent with the current quality assurance policies Education Participates in the training and in-servicing of pharmacists, pharmacy technicians, pharmacy students, pharmacy residents, and other medical center personnel as required Serves as a preceptor for pharmacy students and pharmacy residents;
    • maintains at least minimum ASHP requirements for resident preceptorship Program Management Responsible for developing and managing clinical pharmacy activities and functions with an emphasis on improving quality of medical care for patients;

    Plans and assigns work to be accomplished by clinical pharmacy specialists based upon priorities.

    Participates in the planning and forecasting of the assigned clinical pharmacy program areas by providing the Associate Chief, Clinical Services and Chief, Pharmacy Service with periodic and comprehensive evaluations of the goals, objectives, and prospective changes in functions of the department.

    Advises and supports the Associate Chief, Pharmacy Operations and Chief, Pharmacy Service in planning program resource needs and assists in the growth and development of the Pharmacy Department in all areas of practice.

    Research Works closely with investigators conducting clinical studies and / or medication use evaluations in a variety of areas.

    Aids in the development and initiation of new protocols and procedures with the institution, designed to optimize medical care at the state of art level and strive toward cost effective therapy.
