    Essential Functions

    Patient Navigation:

    • Serve as a bridge between the community and the dental clinic, helping individuals access dental services.
    • Assist patients with appointment scheduling and transportation arrangements.
    • Assist patients with navigating financial resources and assistance programs.
    • Document patient interactions and maintain records for tracking purposes.

    Health Promotion and Advocacy

    • Advocate for the oral health needs of the community, especially those facing barriers to care.
    • Identify systemic issues that affect oral health access and work with the clinic to address them.
    • Distribute educational materials on oral hygiene, nutrition, and preventive care both in a clinical and community settings.

    Referrals and Follow-Up:

    • Provide wrap around service to help patients move between integrated medical and dental clinics and assist patients in making appointments at outside clinics as needed.
    • Patient follow-up may include traveling between clinics or performing home visits.

    Minimum Qualifications

    High School diploma, or equivalency (one year of education can be substituted for two years of related work experience). Demonstrated relationship-building and effective communication skills.

    This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.

    Preferences

    Completion of a CHW Core Skill Training Program and have received a CHW Core Skills Certificate or have 5\ years of work experience as a community health worker. Demonstrated relationship-building and effective communication skills.
