The Community Health Worker (CHW) is a home visitation program responsible for
guiding patients and their families through the maze of community services and
other resources and promoting healthy behaviors. The CHW assists providers
through an integrated approach to care management and community outreach.
Activities that promote, maintain, and improve the health of patients and their
families will be prioritized. CHWs provide social support and informal counseling and
advocate for the health needs of individuals and communities.
Duties and Responsibilities
1. Assists clients in their homes, communities, or clinic settings. Clients/patients
are informed about the program's goals and the potential impact on their
well-being. Assists patients in identifying socioeconomic issues affecting their
overall health and developing health/social management plans and goals.
2. Completes and submits monthly reports and maintains comprehensive
electronic client files that include client notes, information releases,
assessments, and other medical documents obtained on their behalf.
Effectively documents client activities, service plans, and outcomes.
3. Patients are taught how to manage their chronic health conditions and care
for themselves effectively. Assists patients in comprehending treatment plans
and instructions. Encourages patients/clients to take an active and engaged
role in their health and overall well-being.
4. Assists clients in obtaining health-related services, such as obtaining a medical
home, providing instruction on proper medical home use, and overcoming
barriers to obtaining needed medical care and social services.
5. Provides support and advocacy during the initial medical visit or when
necessary to assure clients' medical needs and referrals are being conveyed.
Follow up with both clients and providers regarding health/social services
plans.
6. Travels extensively to client homes, community locations, various
agencies, and other outreach destinations.
7. Performs miscellaneous job-related duties as assigned.
Minimum Job Requirements
● Associate degree, Bachelor's preferred
● Completed degree(s) from an accredited institution above the minimum
education requirement may be substituted for experience on a year-for-year
basis.
Knowledge, Skills, and Abilities Required
● Strong interpersonal and communication skills and the ability to work
effectively with various constituencies in a diverse community.
● Knowledge of community agencies and resources.
● Working knowledge of multi-system outreach programs related to health care
delivery, clinical education, and health-related services.
● Ability to plan, implement, and evaluate individual client care plans.
● Knowledge of transportation and other barriers to care that clients may
encounter.
● Ability to communicate medical information to health care professionals and
care coordinators over the telephone.
● Skill in using personal computers and related software applications, including
e-mail.
● Skill in organizing resources and establishing priorities.
● Creative and analytical thinking.
