    Description

    POSITION SUMMARY

     

    The Community Health Worker will organize and oversee all community health outreach activity for the overall program. He/she will serve as lead project representative to health organizations throughout the region and organize program involvement in health fairs, community events and within non-health care settings. He/she will coordinate study participant recruitment activities oversee training for additional staff and disseminate Program related materials and services. He/she will conduct trainings for volunteer community health workers and follow up on community-based care referrals. He/she will assure that all study protocols are followed and that all involved personnel receive appropriate HIPAA training. He/she will work closely with study personnel on breast cancer screening and clinical trials awareness projects within the overall Program to assure success. He/she will perform other assignments related to the goals and objectives of the program as determined by the agency and the funder.

    PRIMARY ACCOUNTABILITIES

    • Plan, prepare and implement projects and procedures for effective community outreach and enrichment, including training and organization of volunteer community health worker program.
    • Responsible for documentation of project data and statistics.
    • Prepare reports and documents as needed or requested.
    • Facilitate/host community-based workshops, lead community-wide outreach activities and assist with planning of events, including workshops, health fairs and attend staff meetings and trainings.
    • Promote services and programs at the agency.
    • Report to Principal Investigator and Program Director on the status of the project.
    • Promote health education and prevention.
    • Ability to provide leadership & establish commitment to the development and implementation of human resources strategies and initiatives.
    • Demonstrate a high level of skill at building relationships and strategic partnerships
    • Demonstrate strong interpersonal and influence skills.

    • Ability to travel throughout the Region as assigned.

    • Maintain current patient instructional and health education materials for use with patients receiving care as deemed necessary in accordance to UMMAM referral services.

    POSITION REQUIREMENTS

    Education

    • A degree in sociology, human services or other related field from a four-year accredited college is desirable; however, experience in social service, community relations, or related fields may be substituted. A high school diploma is required.

    Experience

    • Knowledge regarding clinical operations preferable
    • Must have strong working knowledge of computers, office automation software and related tools.
    • Demonstrated success with managing encounters to meet overall organizational goals, objectives, and outcomes

    Physical/Environmental

    • Normal accessibility and mobility throughout the region required.
    • Normal overtime/extended work hours.

     

    Additional Requirements

     

    • Position requires sufficient hearing level to effectively communicate with people directly or by telephone.
    • Moderate physical activity. Requires handling average-weight objects up to 15-20 pounds, assisting with patients and standing and/or walking for most of the day.
    • Work involves considerable exposure to unusual elements such as extreme temperatures, unpleasant odor and/or loud noises.
    • While performing the duties of this job, the employee is regularly required to maintain sufficient visual acuity to read, write and operate equipment.
    • Position requires sufficient hearing level to effectively communicate with people directly or by telephone.
    • All other duties as assigned by the Outreach Director
