and Winston-Salem
service area

assist
with admitting new patients to hospice care
. This position will
complete the admission process and develop
an initial
plan of care
.
Duties
- M
eet with the patient and family
to review
hospice service
s
and philosophy and obtain consents for care -
Develop
an initial
care plan that
establishes
immediate goals and interventions, based on nursing diagnosis -
C
ommunicat
e
with hospice staff members and other facilities as needed to coordinate
optimal
care and use of resources for patient / family members
- Provide
direct
nursing and supportive
care
for the terminally ill patient
Schedule
- 3
1
2
-hour shifts
, 10:00am-10:00pm
- Rotating
weekends
Qualifications
- Valid Registered Nurse License
-
BLS
Certification
- Valid Driver’s License
