Performs technical and professional work in orchestrating all case management functions in the development and implementation of chemical dependency treatment services. Performs screenings to establish client eligibility and assessments in order to develop individualized treatment plans. Records client progress and provides group and individual counseling. Maintain working relationship with community agencies and programs to insure adequate case management. Be an effective role model for clients. Serve as a presenter in the implementation of the program specific curriculum. Know and abide by the general provisions of the "Confidentiality of Alcohol and Drug Abuse Patient Records" as found in the General Register, October 1,2003, CFR Part 2 and the HIPPA Act of 1996. Must have your License Chemical Dependency Counselors License.
