OVERVIEW:
This position is responsible in the transcription and documentation of patient medical records based on the Physician’s verbal or written notes. Efficiency, accuracy and detailed data entry are critically essential in this position. Candidate should have the ability to write routine reports, proficiency in typing and can summarize patient information in a clear, complete and accurate manner. Must be trustworthy and must handle information with strict confidentiality.
ROLES AND RESPONSIBILITIES:
Responsible for documenting accurate and detailed information of patient visits including medical history, diagnosis, treatments and care plans.
Gathers data such as nursing notes, prior records, labs and radiology results.
Proofread and edit all medical documents for accuracy, spelling, punctuation and grammar.
Ensures that all clinical data, lab results and other related information are included in the Physician’s progress notes.
Performs clerical duties and tasks as assigned.
QUALIFIFCATIONS:
A. Education
With High School Diploma orA graduate of any course, healthcare field is preferred but we are willing to train the most qualified candidate

At least one (1) year related work experience
