Job Title: Network Bridge Counselor

Hours: Full-time - Remote work possible.

Travel: Position is based in Greensboro but requires significant travel within the network’s region (Alamance, Caswell, Guilford, Montgomery, Randloph, Rockingham, and Stanly counties.)

Position Summary: The Network Bridge Counselor will address the obstacles to care among persons living with HIV who are “lost to care” (dropped out of care and/or are chronic no-shows). The Network Bridge Counselor will strive to re-engage these clients into care and help them access the medical care and support services available. The Network Bridge Counselor will also support individuals newly diagnosed with HIV (or new to the area and living with HIV) by linking them to available resources. The Network Bridge Counselor will engage with clients for 3-6 months helping them get into and remain in care. The goal is to transfer them to a qualified medical case manager for on-going, long-term service and support.

In addition, the Network Bridge Counselor will represent CCHN in various state MCM responsibilities including participating in MCM Advisory Committee meetings and providing network training to new case managers.

Qualifications:

· Minimum of a bachelor’s degree in social work or equivalent social services degree.

· Experience in social work principles, techniques, and practices with application to specific casework.

· At least one year experience with HIV disease care and service; knowledge of basic HIV disease process and its impact on persons living with HIV/AIDS and their families. Lived experience is a plus.

· Experience in mental health and substance misuse treatment.

· Understanding of governmental and private organizations that offer HIV-related resources in the region, working knowledge of governing regulations and policies.

· Ability to respect and non-judgmentally relate to clients from diverse backgrounds including country of origin, age, gender identity, race/ethnicity, sexual orientation, mental health status, chemical dependence, religion, and spirituality.

· Ability to recognize evolving psychosocial issues and respond to changing needs of clients and their families.

· Excellent organizational and communication skills; strong computer skills, including recent experience with Microsoft Office.

· Professional working proficiency in Spanish preferred.

Additional Requirements:

· A valid North Carolina driver’s license with a “safe” driving record.

· Ability to pass criminal background check and basic drug screen.

Pay: $48,000.00 - $51,000.00 per year
