Part Time Patient Care Coordinator

  • GENERAL SUMMARY:

    The Patient Care Coordinator (PCC) is responsible for providing day-to-day administrative support to our hospitalists, allowing our clinicians to maximize time spent on patient care. This role does not involve direct patient care but instead centers around enhancing patient satisfaction, improving communication, and maximizing organizational efficiency and effectiveness.

    DESCRIPTION OF DUTIES:

    • Collaborate closely with the EM and HM Site Medical Director(s), Lead APP(s), Keystone personnel, Program Coordinator, and ARMC personnel;
    • Support several physicians and advanced practice providers (APPs) on an administrative level;
    • Properly distribute patients between clinicians;
    • Create a daily rounding list by 7:00a each day and distribute the final rounding list to clinicians, floors, nursing stations, etc.
    • Respond to all codes, RRT, and Heart alerts to ensure that the Keystone clinician is present;
    • Meet with each HM clinician daily;
    • Provide “on-call” list to floors;
    • Conduct patient visits to all discharges and new admissions;
    • Distribute the Keystone brochure, “What is a Hospitalist?” to all patients;
    • Manage the completion and collection of patient satisfaction surveys at the bedside;
    • Act as the liaison between hospitalists, hospital staff, and Keystone;
    • Answer and maintain the Admit Page;
    • Answer calls from the Emergency Department and outlying hospitals as well as local doctors’ offices;
    • Collaborate with ARMC case management on request;
    • Answer all telephone calls professionally and respond to all e-mails and voicemails before leaving your shift;
    • Function as the transfer center for incoming patients to the Hospitalist Service;
    • Communicate effectively with Bed Management for assigning patient placement;
    • Assist in resolving patient complaints by interacting with charge nurses and patient grievance personnel to communicate effectively with the clinicians;
    • Assist in the completion of patient FMLA paperwork;
    • Assist in completing paperwork received by fax (things that HH, DME companies send over to be filled out, signed, and faxed back);
    • Assist in the completion of questions from the pharmacy;
    • Attend ARMC and Keystone meetings as requested. EX: daily multidisciplinary rounds (MDR), throughput meetings, weekly PCC meetings, etc;
    • Assist with on-site provider orientation;
    • Ensure the Keystone office is always clean and well organized before leaving your shift;
    • Conduct themselves at all times in a manner consistent with and that promotes the following:

    • Perform other related duties as required and assigned

    QUALIFICATIONS:

    • Healthcare-related degree preferred;
    • Five or more years of experience in a hospital setting;
    • Must have strong critical thinking skills;
    • Ability to communicate professionally and effectively both verbally and in writing;
    • Demonstrate strong customer service skills;
    • Ability to maintain confidentiality and security of sensitive information, including patient data;
    • Must be a team player with the ability to balance team and individual responsibilities.

    WORKING CONDITIONS:

    • Must be able to stand for prolonged periods and walk up to 50% or more of the work day;
    • Hospital environment;
