As the Hospice DPCS, you will oversee the overall operations of the hospice program. This includes managing patient care, coordinating with healthcare professionals, and ensuring compliance with all regulatory requirements.
Duties:
- Provide leadership and direction to the hospice team
- Develop and implement policies and procedures to ensure high-quality patient care
- Coordinate and manage the admission and discharge process for hospice patients
- Collaborate with physicians, nurses, and other healthcare professionals to develop individualized care plans
- Conduct regular assessments of patient needs and adjust care plans as necessary
- Provide education and support to patients and their families regarding end-of-life care
- Ensure compliance with all state, federal, and accreditation regulations
- Monitor and evaluate the performance of staff members
- Participate in quality improvement initiatives to enhance patient outcomes
Skills:
- 4 to 5 years of progressive hospice leadership responsibility for directing regulatory compliance with Medicare and Department of Public Health for a hospice organization.
- Required extensive experience with and compliance to the hospice Medicare Conditions of Participation.
- Proficiency in using electronic medical record systems since MatrixCare is the EMR system used
- Experience with ADR and regulatory audit process.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Strong organizational skills to manage multiple tasks and prioritize workload effectively
