DIABETES EDUCATION PROGRAM COORDINATOR
MINIMUM EDUCATION REQUIRED:
Graduate of a Nursing program or Bachelors of Science in applicable field.
MINIMUM LICENSURE/CERTIFICATION REQUIRED:
Current License as a Registered Nurse (RN), Registered Dietitian (RD), Certified Diabetes Educator (CDE), or Diabetes Care and Education Specialist (CDCES) required
ADDITIONAL QUALIFICATIONS:
WHAT YOU'LL DO:
As a Diabetes Education Program Coordinator, you will need to be a registered professional nurse or dietitian with advanced knowledge of diabetes and clinical expertise. You will be responsible for oversight of the planning, implementation, and evaluation of the Diabetes Self-Management Education (DSME) Program, curriculum, and continuous quality improvement process, as well as establishing consistency of diabetes care in the outpatient setting. The diabetes educator will counsel patients and their families on medical nutritional therapies, diabetes medication management, and the need for lifestyle and behavior modification in both the individual and group patient setting. As the Coordinator, you will ensure that the National Standards are met and always maintained. You will coordinate the operational goals of the Diabetes Education Program, provide tools and skills for homecare to patients and families, and make recommendations to staff for changes in diabetes management. PROGRAM LOCATION:
The Diabetes Education Program Coordinator will be over the diabetes education in multiple outpatient clinics in the central/south Georgia area. Some travel between locations will be required. Travel costs will be covered
