 Purpose of this position

This position provides support services and crisis resolution to the child, parents, guardians and families of youth in foster care to enhance, assist or facilitate the child and family’s ability to function successfully within the family and in the community upon discharge.  

Assigned work hours 

8:30am-5:30pm Mon-Fri with flexibility to meet client needs. (evening and weekends may be necessary)

 Qualifications 

MSW and two (2) years clinical /related experience preferred. Bachelor’s Degree in related field with relevant experience considered. Must have a Valid NYS Driver’s License. 

Delegation of authority 

Authority is delegated to the individual in this position to: Program Manager

Salary Range- $20.50 Min - $31.01 Max.

Job Description

• Provides services to parents, guardians and families having therapeutic youth in foster care within the 8 counties of Western New York. The actual area will fluctuate dependent on case admissions and need.
• Enhance child’s ability to function as part of a family/caregiver unit.
• Enhance family/caregiver’s ability to care for the child within the home or community.
• Provide child and/or family support, guidance, mentoring, coaching and/or training to enhance successful functioning in the home, school or community within context of the child’s circumstances.
• Provide planned short-term relief for family/caregivers of therapeutic foster child.
• Provide emergency short term relief for family/caregivers of SED or DD child to resolve a crisis and to help the child return to successful functioning.
• Assist the child, family/caregiver, and community/school staff with understanding the child’s needs and provide techniques and information to better respond to the needs. 
• Develops family interventions to be incorporated into the treatment plan of assigned cases.
• Prepare child to engage and facilitate success in any paid work environment.
• Provide opportunities and training for independent living skills to child age 14 or older as they perform in a work setting.
• Skill builder will complete the independent life skills assessment with youth. 
• Provides support and updates for collateral contacts as needed. Communicate regularly with collateral contacts. Document in connections as necessary. 
• Educate and train child and/or family/caregiver to address specific issues that disrupts or jeopardizes the child’s successful functioning and or placement.
• Develop behavior modification plans, crisis safety plans to aid in the successful functioning of identified clients on caseload.  
• Submits paperwork in a timely and accurate manner.
• Prepare and develop visitation plans with biological parents and county caseworkers as outlined by court and service plan.
• Conducting supervised visitation with youth and their families.
• Documentation of all visits in clinical notes and progress notes.
• Participate as a team member in the progress of the visitation plan and Treatment plan. 
• Maintains appropriate liaison with families, colleagues and staff of GA and referring agencies.
• Is flexible and works to solve problems in a positive manner.
• Travels extensively using own vehicle.
• Operates basic office equipment such as fax, computer, copier, cell phone, printer, personal vehicle and agency vehicle.
• Demonstrates sensitivity to the cultural and social-economic characteristics of persons served.
• Assists other program staff as needed.
• Possess a valid New York State driver’s license and to transport youth to and from destinations consistent with the services offered by the agency.
