Establishes accessible systems that respond to patient needs for social services including counseling, financial, bereavement, and specialized support during and after hospitalization. Being a Top Social Services Executive monitors usage and outcomes to continuously improve the service offerings and to offer additional services based on patient needs. Establishes relationships with outside agencies and charitable organizations to leverage additional sources of support for patients. Additionally, Top Social Services Executive requires a master's degree of Social Work or counseling. Typically reports to top management. We have a new opportunity for a full-time Social Services Coordinator at the Mt. Olive Community in our Housing Department!
This role will actively establish links to community agencies and service providers for program participants. This person will also provide basic care management and referral services to participants in the program.
Required Experience:
Education/Training: MSW, BSW, LSW or High School Diploma and 5 years in social work field
Skill(s): strong case management skills, listening, writing, analyzing, empathy, organized, confidentiality
Experience: Two of more years of social work experience with degree or 5 years in social work field. Working knowledge of supportive services & resources for the elderly population. Strong case management skills.
