Requires a bachelor's degree in area of specialty or in a related area, and may require state licensure. Additionally, Trader (Options) typically reports to a manager or head of a unit/department. To be a Trader (Options) typically requires 2 to 4 years of related experience. Gains exposure to some of the complex tasks within the job function. Care Manager Options Counselor

  • To conduct comprehensive assessments of all referrals and individuals requesting Long Term Supports and Services. To advocate on behalf of SWIRCA’s clients to obtain needed services/assistance. To evaluate, coordinate, monitor and follow-up on Home and Community Based Services for individuals as directed by the Supervisor.

    Main Responsibilities

    • To conduct in-depth face to face assessments of individuals’ challenges, resources, functioning, needs and to develop a written “person centered plan of care” in conjunction with the individual and their chosen representative as requested.
    • To complete all assessment, waiting list and care planning documents as required by SWIRCA and State Guidelines.
    • To provide information to individuals and their representatives concerning all potential service delivery methods including private pay, formal supports, informal supports or government funded services. To help reflect what services and supports could be available to the person so they can attain their “vision” of a good life and achieve their goals.
    • To provide individuals and/or families with accurate and complete information regarding the extent and nature of the services available to them.
    • To coordinate service delivery with community resource agencies.
    • To make every effort to foster maximum self-determination on the part of the individual.
    • To assist with the intake and support of Money Follows the Person recipients as they transition from the Nursing Facility into the community.
    • To assist with Pre Admission screenings for those seeking placement from home when the Community Transitions TL is not available.
    • To maintain professional standards of individual confidentiality. To discuss cases only for professional purposes and only with persons directly and professionally concerned with them.
    • To complete daily, monthly and quarterly reports and documentation as required.

    Qualifications

    Baccalaureate Degree in Social Work, Sociology, Psychology, Gerontology, Health & Human Services or Registered Nurse licensed to practice in the state of Indiana and an Indiana Case Management Certification preferred. Any Baccalaureate Degree acceptable. Experience assisting the aged and disabled populations preferred. Must be proficient in Microsoft Word, Excel and Email. Familiarity with a variety of the field’s concepts, practices, and procedures helpful. Current Driver’s License and use of Automobile.
