Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. General Duties and Responsibilities:
· Coordinate scheduling of care plan conference following the RAI manual and any applicable state guidelines.
· Work with staff so as to enhance reimbursement, as applicable through documentation of care and services provided.
· Understands, complies with, and promotes all rules regarding resident rights and confidentiality.
· Attends and participates in orientation, training efforts, educational activities, in-service functions, and staff meetings, including educational programming related to the MDS assessment process.
· Ensures MDS records are submitted in a timely manner in accordance with applicable RAI and state guidelines.
· Coordinates the facility resident review process.
· Coordinates the facility “triple check” for skilled residents monthly.
· Performs random audits of skilled resident records for presence of documentation to support skilled services received.
· Monitor facility CMI (case mix index).
· Inform residents, responsible parties and staff of Medicare/managed care coverage status through appropriate documentation in accordance with all federal, state, and facility guidelines. Provides updates as necessary.
· Provide appropriate notice of non-coverage for Medicare Part A and/or managed care providers.
· Scheduling of all PPS and/or managed care MDS assessments in accordance with applicable RAI and state guidelines.
