Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The Transplant Nurse Coordinator gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. Primary Responsibilities
The RNCC partners with the Texas Independence Health Plan Institutional Special Needs Plan (ISNP) Nurse Practitioner (Plan NP) to coordinate care for ISNP Beneficiaries (Plan Members), maintaining routine contact with Plan Members, family, and facility staff, and informing Plan NPs and/or facility staff when there are potential medical, emotional, or behavioral changes in a Plan Member’s condition. Functions as a Case/Care Manager and patient healthcare navigator for Texas Independence Health Plan Members within nursing facilities. Weekend on-call support duties, and all other duties as assigned.
