Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. POSITION SUMMARY:
The Care Coordinator is clinically responsible for the coordination of new and established patients within the Clinics. This position will work in partnership and collaboratively with all members of the Health Care Team to assure timely access to services, resources and provide continuity of care. The Care Coordinator helps to ensure the patient understands every aspect of their care. This position is expected to work within the program directives set by funding sources within the philosophical framework set by the Board of Directors.
EDUCATION/EXPERIENCE:
1. High School Diploma required
2. Associates/Bachelor’s degree in nursing from an accredited registered nursing program and licensed as a Registered Nurse in the State of Hawaii
3. 5 years clinical experience in an outpatient clinic preferred
4. 2 years of leadership experience preferred
5. Proficient in the following computer skills: word processing, entering data onto and sorting data in spreadsheets, navigating through the internet, email, and experience with electronic health records
6. Communication skills (verbal and written) required
7. Ability to work in partnership with community health organizations
8. Ability to communicate effectively with staff members, community members, and patients
9. OSHA training annually
10. BCLS every two years
