Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The Transplant Nurse Coordinator gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. The Clinical Transplant Coordinator fulfills the primary responsibilities involved coordinating the multidisciplinary approach in the evaluation, treatment and facilitation of the transplant process, and the follow-up care of those individuals receiving organ transplants, and those who have donated. The role is one of collaboration and coordination and is both collegial and consultative to nursing leadership, primary nurses and other members of the health care team.
