Transplant Nurse Coordinator jobs in Nashville, TN

Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The Transplant Nurse Coordinator gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work.     Job Summary:
    JOB SUMMARY
    Coordinates and supports individual patients and families throughout point of care program(s) to ensure program goals are effectively executed with some guidance. Assures coordination of diagnostic evaluations, treatments, surgeries and follow-up appointments throughout the course of care/episode for a specific patient population.

    KEY RESPONSIBILITIES
    • Partners with clinical care staff and providers to ensure all regulatory requirements are met to maintain patient care needs, program guidelines and/or required accreditation standards.
    • Acts as a point of access for patients, referring providers and the Vanderbilt community.
    • Develops both short and long term goals to ensure programs' success.
    • Provides support to patient care team to market programs and streamline business models to allow for appropriate growth.
    • Establishes contact with existing, newly diagnosed, potential, or referred patients by facilitating access to providers, educational materials and communication with care providers.
