Transplant Nurse Coordinator jobs in New Bedford, MA

Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The Transplant Nurse Coordinator gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work.     • Completed a Certified Nursing Assistant (CNA) training program and have an active license
    • Active CPR Certification
    • 1 year of relevant experience as a CNA

    • The ability to handle situations with confidentiality and maturity
    • The ability to read, write, and speak English
    • Commitment and dependability
    • Rotating weekend and holiday on-call responsibilities

    Primary Job Responsibilities:

    • Care for patients in their homes in conjunction with the rest of the clinical team.
    • Follow a personalized care plan for each patient that is assigned by the case manager who is available to support you in your responsibilities.
    • Assist with activities of daily living (ADLs), light meal preparation, companionship, respite when applicable.
    • Promote a safe environment and assist with ambulation/transfers.
    • Maintain communication with the interdisciplinary team and the patient’s caregivers.

    All employees must be vaccinated with booster for COVID-19.

    Job Type: Full-time

    Pay: From $18.00 per hour

    Expected hours: 40 per week
