Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The Transplant Nurse Coordinator gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work.   • Position Description: Position Overview:
    This employee assists the Clinical Manager and the HFA Nurse Coordinator in assuming management responsibility for the delivery of services for the Healthy Families America Program. This employee is responsible for ensuring the completion of comprehensive family intake assessments, facilitation of group reflective consultation, provision of direct services, supervision of staff and community collaboration. Understands and commits to the agency mission. Performs all duties and responsibilities listed herein, as well as other duties that may be assigned.

    Essential Duties and Responsibilities:
    Complies with all federal, state, and local laws and regulations as appropriate for the position. Complies with all policies and procedures of the agency, including those specified in the employee handbook.
    Meets all documentation and productivity standards as identified by the agency.
    Provides leadership, guidance and consultation to the Parenting Support Team and works collaboratively to ensure practices which enhance quality client care and promote a customer focused environment.
    Orients, trains, supervises, and evaluates HFA staff, assisting with competency review of staff as needed.
    Acts as point of contact for new referrals and conducts initial phone screening when necessary.
    Oversees the completion and documentation of intake assessments for clients participating in HFA.
    Participates in weekly agency and interagency meetings, assisting the Clinical Manager and HFA Nurse Coordinator in reviewing the work of the HFA team, ensuring fidelity to program policies and strength based, client centered approach.
    Facilitates case management discussions in team meetings, ensures appropriate decision-making regarding client needs and referrals to specialty teams and internal/external services.
    Strengthens and maintains effective working relationships with clients, agency personnel, community resources, and liaison with agencies as appropriate.
    Interacts effectively with multidisciplinary staff and with other providers involved in the care of clients.
    Supervises HFA team members utilizing reflective practice. Addresses interpersonal barriers and strengths in personnel, empowering those receiving supervision to offer guidance in a constructive manner.
    Prepares for and facilitates group reflective consultation for HFA staff, if a certified Reflective Group Consultant.
    Respects and maintains client confidentiality.
    Meets all program evaluation and reporting requirements.
    Contributes to the development and revision of program policies, procedures, program development and grant opportunities.
    Assists Clinical Manager and HFA Nurse Coordinator in maintaining appropriate staffing, distribution of workload and staff productivity. Manages a partial client caseload at times.
    Makes hiring recommendations, collaborates with HFA Nurse Coordinator, Clinical Manager, VP and/or Human Resources when utilizing the disciplinary process, or making decisions regarding promotion or termination.
    Contributes to employee satisfaction through positive interactions and professionalism.
    Promotes a positive working environment for program staff and the agency.
    Promotes the growth of the VNA by representing the organization, its mission, and its role in the community in a professional manner.

    Education and/or Work Experience Requirements:
    Masters degree in social work, human services, or fields related to working with children and families, or bachelors degree in social work, nursing, or related fields with 3 years of relevant experience, or less than a bachelors degree but with commensurate HFA experience.
    If RN, an active license to practice as a Registered Nurse in the State of Nebraska.
    If MSW, Licensed Social Worker or a Mental Health Practitioner, preferred, by the State of Nebraska. Endorsement in Infant Mental Health preferred.
    Three years experience in provision of social work and/or nursing services. Knowledge of social work and/or nursing theory and practice. Knowledge of the principles and practices of community health. Knowledge of community services. Prior supervisory or management experience preferred.
    Bilingual in English-Spanish preferred.
    Certified in Basic Life Support.
    Experience in home visiting with a strong background in prevention services to the 0-5 age population with knowledge of infant and child development and parent-child attachment.
    Experience in, or a solid understanding of: supervising and motivating diverse staff with humility, as well as providing support to staff in stressful work environments, experience with family services that embrace the concepts of family-centered and strength-based service provision, knowledge of maternal-infant health and dynamics of child abuse and neglect, experience in providing services to support culturally diverse communities/families, experience with reflective practice, preferred, and experience working with children and families.
    Ability to interact with a multi-disciplinary team and other providers involved in the care of clients. Must be task-oriented with the ability to complete a project/task within a specified time frame.
    Ability to establish trusting relationships, accept individual differences, have knowledge of infant and child development and be open to reflective practices.
    Willingness to engage in building reflective practice (e.g., capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision, etc.)
    Understands legal and ethical obligations for maintaining professional competency. Participates in professional development activities. Integrates standards into practice. Limits practice according to competence.
    Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers. Ability to present information and respond to questions from employees, clients, managers, and public.
    Demonstrates sensitivity to the service populations cultural and socioeconomic characteristics.
    Excellent computer proficiency. (MS Office Word, Excel, and Outlook)
    Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.
    Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
    Ability to solve practical problems and deal with a variety of standards effectively. Ability to interpret a variety of instructions furnished in written or oral form. Ability to prioritize, check for errors, and spot inaccuracies. Ability to predict the logical outcome of various choices and to make good decisions.
    Adapts to changes in procedures and technology. Willingly learns new skills as required for the position.
