Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The Transplant Nurse Coordinator gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work.     This position will provide coordination and case management for ventricular assist devices (VAD) and cardiac transplant patients in collaboration with the program medical director and referring physicians to promote optimal patient outcomes. Responsible for clinical and technical support for ventricular assist devices or cardiac transplant patients regarding evaluation, implant management, post follow-up and clinical training. Effectively uses care conferencing and multidisciplinary transplant/VAD committee for patient evaluations and clinical reviews. Facilitates patient care within an outpatient clinic and shares 24 hour call. Will coordinate patient care through medical procedures, ongoing education, inpatient and outpatient care delivery and follow-up.

    Required qualifications:

    • Education to meet certification, license or registration requirement.
    • Oregon Registered Nurse License upon hire.
    • VAD coordinator experience

    Preferred qualifications:

    • 3 years of Clinical experience with VAD patient care.
