Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board.     Organ Acquisition Coordinator – Duke University Hospital

     

    Duke University Health System- Duke University Hospital seeks to hire an experienced Organ Acquisition Coordinator who will embrace our mission of Advancing Health Together.

      

    Summary

     

    Clinical Practice

    1.The organ acquisition coordinator is a registered nurse trained in the evaluation of deceased donor organ offers, waitlist review and patient selection, donor-recipient matching, informed consent procedures for organ offers, and coordination of procurement and transplantation procedures.

     

    2. The organ acquisition coordinator works either remotely or in a hybrid role on a team that is responsible to coordinate all aspects of the organ transplant process 24/7 including nights, weekends, and holidays.

     

    ***Shift schedule will be three 12 hour shifts per week, rotating between 7am-7pm and 7pm-7am.

     

    3. Ability to apply knowledge of the United Network for Organ Sharing (UNOS)/Organ Procurement and Transplantation Network (OPTN), the Centers for Medicare and Medicaid Services (CMS) Condition of Participation (CoP) for Transplant Programs to daily work is required to ensure hospital and transplant program adherence to all regulations.

     

    Work Performed

    • General Practice receives all organ offers through the OPTN/UNOS Donor Net Online Portal.
    • Requires disposition of initial organ offers to include an acceptance or turn down with appropriate refusal codes within 60 minutes of electronic offer.
    • Requires disposition of primary offers and expedited offers to include an acceptance or turn down with appropriate refusal codes within 30 minutes of electronic offer.
    • Reviews and interprets the organ match run to identify the appropriate NCDU recipients and screens offers based on transplant center specific criteria.
    • Understands and effectively communicates waiver information granted if applicable for each import organ, understanding the type of waiver and implications for payment of the organ.
    • Presents critical aspects of the organ offers to the transplant surgeon focusing on donor history, hospital course, medications, major treatment interventions, laboratory review including serology, and blood type.
    • Requires clarifying communication between the offering Organ Procurement Organization (OPO) and information listed in Donor Net to obtain complete clinical, psychosocial, infectious disease, laboratory, and logistical information on the organ donor and specific organ offer.
    • Provides advanced patient selection assistance to the transplant surgeon and assists in reviewing, screening, selecting, and recommending transplant candidates based on organ specific criteria.
    • Coordinates the recovery of the organ to include transportation logistics (ground and air), timing, and procurement team selection including surgeon, procurement coordinator, preservationist, and fellow.
    • Communicates pertinent donor-recipient matching criteria such as infectious disease screening results, acute donor behavioral risk factors and other factors critical to the patient pre and peri-operative care plan.
    • Coordinates the transplant event process with includes patient hospital admission set up, booking recipient OR case and clinical team notifications.
    • Detailed documentation of all communications and decisions related to organ offers and their acceptances are required on a tracking tool or database.
    • Reviews organ specific information that may impact patient decision making, provides education to patients and answers questions during organ offer calls, and ensures there is informed consent to accept a specific offer understanding the risks-benefits.
    • Performs a nursing assessment of potential candidates to determine readiness and medical, psychosocial, and financial suitability to undergo transplantation at time of organ offer. After the transplant event, removes patient from the UNOS waitlist within 24 hours.
    • Completes all documentation for transplant event within the medical record to include Epic Phoenix tab, protocols, checklist items and appropriate clinical team notifications.
    • When possible, attends the organ specific listing committee meetings and/or waitlist management meetings to better familiarize themselves with patients toward the top of the organs transplant lists that will be presented an organ offer.
    • At the direction of the transplant provider after hours, the organ acquisition coordinator will be responsible to update urgent patient waitlist status values and add urgent patients to the waitlist.
    • Receives, evaluates, and triages all pre- and post-transplant patient calls after hours.
    • Performs nursing assessment by phone and provides directions to patients.
    • Consults with MD/clinical team as clinical protocols determine.
    • Receives positive donor culture notifications from OPTN/UNOS.
    • Documents and communicates positive culture results to appropriate clinical teams.
    • Facilitates reporting potential donor derived adverse events through the UNOS portal.
    • Participates in quality assessment and performance improvement activities including but not limited to data entry, data abstraction, data analysis, report compilations, QAPI Committee presentations, participation in improvement teams.
    • Professional Leadership and Development Role models advanced practice professional nursing through conduct, appearance, communication, mutual respect, leadership, ethical decision- making, critical thinking, and problem-solving skills.
    • Communicates effectively to maintain positive working relationships and achieve desired outcomes.
    • Supports and participates in the strategic plan for the Duke Transplant Center, Duke University Health System, Department of Nursing, and Department of Advanced Practice Nursing.
    • Demonstrates responsibility for professional practice through active participation in professional organizations, a minimum of 25 continuing education hours/year, and active participation in CSU and APN departmental meetings/educational opportunities.
    • Enhances the body of knowledge in nursing and solid organ transplantation through exchange of ideas and knowledge using venues such as posters, newsletters, presentations, and publications.
    • Cross coverage as needed for other team members for PTO, meetings, etc. Adhere to a schedule to ensure customer availability and demonstrate flexibility to schedules according to patient or call volume or staffing needs. Perform other related duties incidental to the work described herein.

     

    Knowledge, Skills and Abilities

    • Attention to detail and effective communication and documentation with numerous physicians, organ procurement agencies, Operating Room (OR), Intensive Care Unit (ICU) and Emergency Department (ED) staff, the patient and the patient's family are required.
    • Ability to make decisions, problem solve and prioritize tasks effectively in a fast-paced environment. Ability to work the shift and hybrid (onsite and remote) requirements of this role.

     

    Minimum Qualifications

    Minimum Education Required: BSN.

    CCTC or CPTC certification preferred.

     

    Required / Preferred Experience

    *A minimum of 3 years of clinical bedside RN experience is required.

    *Previous experience within Transplant, Critical Care, or Chronic Disease management highly preferred.

     

    Degrees, Licensures, Certifications

    Requires current North Carolina Registered Nursing Licensure.
