Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board.     Responsibilities
    Transplant Coordinators in the Bone Marrow Transplant department at UC Davis Medical Center provide care to adults with a variety of medical surgical needs. The Transplant Coordinator II (TC 2) is a nursing resource in the care of the pre and post-transplant patients with a focus in autologous, allogeneic, and CAR-t transplants. The TC 2 has responsibility for the clinical case management of adult stem cell transplant candidates from the time of referral to transplant, and through survivorship. This responsibility involves coordination of the interdisciplinary medical care delivery system. The TC 2 functions as the liaison between the transplant team and the patient, family, medical staff, referring physicians, donor agencies and insurance companies. The TC 2 applies advanced patient assessment skills and knowledge in the area of hematology/oncology, and performs a wide range of duties allowed by The Nurse Practice Act. The TC 2 is accountable for their own practice while providing direct patient care to a diversified patient population.

    The Transplant Coordinator II identifies and implements nursing interventions and evaluates results for a wide variety of populations, directs and acts a resource to other members of the nursing team as assigned and promotes compliance and support for policies. The primary responsibilities for this position consist of providing total, comprehensive, continuous, patient-centered care which includes: assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care. Other responsibilities include delivering direct patient care, ensuring patient satisfaction, training and delegating duties, and adhering to University policies and procedures as well as federal, state, and local regulations.

    Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care.

    Required Qualifications
    • At least two (2) years of clinical RN experience in Oncology/Bone Marrow Transplant
    • Current AHA CPR/BLS Certification
    • Verification of graduating from an accredited school of nursing
    • Possession of valid California RN license without restrictions.
    • Commitment to excellence in patient care
    • Knowledge of legal implications in clinical practice and documentation
    • Knowledge of performance improvement and evidence-based practice

    Special Requirements
    • Must demonstrate ability in assessment, planning, implementing and evaluating patient care, taking into consideration protective interventions, development (behavioral and physical) and physiological characteristics specific to the patients
    • Ability to safely lift 25 pounds and assist in moving patients, and to stand on feet over extended periods.
    • Works overtime as defined by the collective bargaining agreements, floats to other areas and works weekends and holidays.
