Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The donation advocate's primary responsibility is to advocate for organ, eye and tissue donation through notification of donation intent with potential organ donor families or obtaining authorization for donation from donor families on potential donors who have not declared their donation intent. The donation advocate will also obtain, when able, a past medical social history. Information is to be articulated in an easily understood and accurate manner to families whose ability to process information is impaired by emotionally traumatic circumstances. Coordinates and facilitates emotional support during and after the discussion of the donation process, remaining aware and sensitive to diversity of all kinds (cultural, religious, educational, etc.) while working with families and hospital staff. Primary first responder to donor referrals from partner hospitals. Responds to referrals, as outlined, in standards of practice within LifeQuest policies. Position requires extensive monthly on-call duties for 24/7 referral response to hospitals as determined by the director of donation advocacy, manager of donation advocacy or other members of senior leadership.
