Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The Transplant Nurse Coordinator gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. Position Summary: Reviews and coordinates all needed clinical information prior to patient admission to ensure the perfect pathway for new patients and to ensure an exceptional customer service experience for the patient. Communicates with branches and teams regarding new patients and maintains positive, professional relationships with all internal and external customers. Per Diem Role; Weekend and weekday availability required. Current LPN License required. Remote role, but need to be able to report to office in Worcester.
Experience:
Minimum 2 years related experience including medical/surgical and community health and/or experience with a managed care organization. Current LPN license required.
Previous intake, liaison or discharge planning experience is preferred.
Knowledge of home care standards and practices required.
Knowledge of medical terminology and treatment modalities of patient diagnoses required.
