  • Job Purpose The Pua Liliʻu Wellbeing Practice Model forms the foundation for all direct service practice within LT.  Rooted in Hawaiian cultural foundation and developed through an iterative organizational process, it is a holistic model that represent LT’s interpretation of the most important elements that define and maintain well-being.  Pua Liliʻu provides alignment for all LT frameworks and manuals that guide our direct service practice.   The Pua Lili‘u Manager (PLM/ Case Manager) supports and navigates youth and young adults services and programs for Lydia House. The role ensures planning and execution of the services and activities, partnerships and collaborations supporting the kamali‘i for the Engagement Center at LH, and provides all supports necessary for successful achievement of “Thriving Plan” for the qualified and assigned young adults in the “Thriving Supportive Program (TSP).” This position will support outreach, low-barrier access to services, and a safe place to and for youth and young adults in the Engagement Center. The PLM will oversee and provide a caring adult presence, navigation, program delivery and coordination, and coaching for the youth in the TSP. The PLM is responsible for researching and incorporating culturally-centered interventions according to the Pua Lili‘u framework and matching it to the needs of the youth and young adults. This role utilizes knowledge of youth best practices, community outreach and collaboration, and constant opportunities to acquire and utilize feedback from kamaliʻi and to constantly enhance and improve services to meet youth needs.   Essential Responsibilities Engagement Center Promotes a Safe physical and emotional environment for participants and staff, with low-barrier access for all youth and young adults (YYA) qualified to enter the center. Supports and develops services and activities which meet kamaliʻi basic needs and stabilize their lives, in a youth-centered approach. Collaborates with and supports learning for LH teammates, and external partners, to support youth best practices and approaches tailored to the needs of kamaliʻi. Promotes an environment where kamali‘i may feel safe to self-direct their activities, readiness and degree of support for stability, sense of belonging, and future orientation. Performs community outreach, analyzes environment surrounding community, and gathers feedback from partners, kamaliʻi, and other community members to incorporate into the design of services provided. Collects and reports on participant data and collaborates with other teams (ie. Social Services, Research and Evaluation) to further utilize this information in the understanding of demographics needs and services to inform impactful, transformative models. Report and work collaborative to the lead PLM and Director to support all activities within Engagement Center.     Thriving Supportive Program Outreaches and promotes YYA from Engagement Center who might be interested in enrolling in TSP. Assesses fit and readiness for best success of YYA to enroll and participate in TSP. Provides best, positive opportunity for successful completion of  kamali‘i “Thriving Plan”, especially in education and workforce reconnection by consistent communication. Supports the assigned case load of YYA in TSP by frequently meeting and seeking success in reaching self-determined goals. Collects and reports participant data continuously, maintaining individual periodical assessment of progress and notes in management system. Contributes to LT’s success through collaboration, communication, and knowledge-sharing with all teammates to improve team and organizational results. Other duties as required.   Other duties: Contributes to the Trustʻs success by accepting new assignments, helping team members, learning new skills, and striving to improves team and organization results.   Qualifications Bachelor’s degree in education, human/social services, or other relevant field. Or have at least 6 years of relevant experience. At least 2 years experience in planning and implementing youth programs. Knowledge of best youth practices (positive youth development) and the ability to selfcare and work with the most vulnerable population.   Job Competencies Collaboration: Ability to interact effectively with youth, community partners, and multi-disciplinary teams.  Knowledge of community resources and services.
