Seasons Change Home Health Care & Hospice, Inc. is looking for a compassionate and dedicated Registered Nurse to join our team! The ideal candidate is an experienced Nurse who has a passion for caring for the elderly and will work efficiently in a fast-paced, home-environment. Providing care to your patient in their home allows you to provide one-on-one care to each patient, and direct your own schedule.

As a Registered Nurse with our agency, you will enjoy working closely with other caring, experienced and dedicated skilled care professionals, as well as our friendly and knowledgeable administrative support staff.

Responsibilities:

· Provide home-based care to patients with illnesses, injuries and chronic diseases – often after discharge from a hospital, rehab or nursing facility. Includes assessment, planning, implementation, evaluation and documentation of care provided.

· Assess patients for home care and hospice eligibility and explain services to patients, families and caregivers.

· Collaborate with Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information.

· Provide supervision for Home Health Aide and LPN as needed/required by the agency policy and/or State/Federal regulation.

· Educate patients on their plan of care, prescribed medication, therapy, diet and exercise.

Requirements:

· Graduate of an accredited nursing program and active RN license in Michigan

· Nursing experience in home healthcare and/or Hospice setting preferred

· Current CPR certification

· Valid driver’s license and reliable transportation

Pay: $1,500.00 - $6,500.00 per month
