Utilization Review Manager - Home Care jobs in Ventura, CA

Responsibilities:

- Provide skilled nursing care to patients in their homes

- Conduct comprehensive assessments of patients' health status

- Develop and implement individualized care plans

- Administer medications and treatments as prescribed by physicians

- Monitor and evaluate patients' progress and make necessary adjustments to the care plan

- Educate patients and their families on managing their health conditions

- Collaborate with other healthcare professionals to coordinate patient care

- Document all patient interactions, interventions, and outcomes accurately and timely

Skills:

- Strong background in pediatrics, behavioral health, and acute care

- Excellent patient service skills with a compassionate and caring approach

- Proficient in conducting vital signs assessments and interpreting results

- Knowledge of physiology and ability to apply it in patient care

- Experience in diagnostic evaluation and interpretation of results

- Ability to work effectively in a fast-paced environment, such as the ER or ICU

- Familiarity with managed care principles and practices

Qualifications:

- Licensed Vocational Nurse (LVN) license in the state of California

- Minimum of 2 years of nursing experience, preferably in home health or a related field

- BLS certification required; ACLS certification preferred

- Strong communication and interpersonal skills

- Ability to work independently and as part of a multidisciplinary team
