Hospice and Visiting Nurse Services Coordinator
Full-time (36 hours/week). Weekday, day shifts. Department call rotation for evenings, holidays, and weekends.
The Hospice and Visiting Nurse Services Coordinator is a supervisory position with the skills and knowledge of a registered nurse. This role is responsible for planning, coordinating, and implementing the care of Hospice and Visiting Nurse Service patients. Promotes the growth and development of the Hospice/ Visiting Nurse departments, ensuring compliance with Federal and State regulations.
Must be able to work with terminally ill patients and their families to help them effectively cope with dying. Experience in Hospice is preferred. Prior experience in supervisory capacity preferred. Demonstrates knowledge in caring for patients and families in death and dying situations. Understands and possesses knowledge of chronic pain management. Possesses an understanding of Hospice concepts and supports the value of palliative care. Able to work with an interdisciplinary team to provide bereavement services. Responds effectively in emergency and stressful situations. Communicates frequently with coworkers, physicians, patients, and families. Ability to arrange and rearrange daily schedule, taking into consideration the duration and location of scheduled visits, meetings, unexpected situations, etc. Reviews and completes chart audits. Is comfortable with independent decision-making and discussing death and dying issues. Keeps current in advances in Hospice and Visiting Nurse Services by attending continuing education programs and in-service as allowed by hospital finances.
