The Nursing & Rehabilitation Center is 460-bed facility providing services for short-term rehabilitation and long-term skilled nursing care, home care programs, a ventilator dependent care, palliative and hospice care, a Memory Care Unit and on-site dialysis. Lindner Residences assisted living community, and Fountaingate Gardens independent living community located on the Commack campus.

Our Assisted Living Community is currently seeking a full time day shift  DIRECTOR OF RESIDENT SERVICES who will embody the passion and commitment required to continue our reputation of providing the gold standard of excellence in assisted living.

Requirements include:

• Current valid NYS LPN license with knowledge of geriatric nursing.
• Basic First Aid Training required.
• Adherence to the Department of Health Regulations as they pertain to Assisted Living Facilities.
• Previous supervisory experience.
• Excellent organizational skills; detail-oriented.
• Ability to work independently in an active Assisted Living Community.
• Exceptional interpersonal skills, coupled with strong relationship building and be an effective leader.
• Commitment to the well-being of our residents; compassion and respect for their concerns.

Responsibilities include, but are not limited to:

• Coordinate and manage all Nursing and Assistive Living Aides.

• Review Medical Evaluations prior to admission.

• Responsible for personnel in the department with respect to hiring, terminating, counseling and performance evaluation.

• Performs in-services as required.

• Apprize family of changes in resident health and behavior.

• Supervise daily logs and verify that they are being maintained by all shifts.

• Responsible for training.

• Responsible for completing the individualized service plans and also the 4397B (Assisted Living Resident Evaluation).

• Responsible for the daily operations of the Wellness Department.

• Responsible for cultivating and motivating a successful team environment.

• Develop new initiatives, programs, and cost-effective measures.
