  • The Tuscarawas County Board of Developmental Disabilities (TuscBDD) arose from a grassroots effort by parents and concerned citizens in 1955 to provide educational services to 12 children with developmental disabilities through the creation of Starlight School. Over time, Starlight School has evolved into TuscBDD, which is a publicly funded county governmental entity offering an array of programs for over 500 eligible individuals. We officially became Tuscarawas County Board of Developmental Disabilities in October 2009.

    We are here to ensure that these residents have available to them the programs, services and supports necessary to help them enjoy a fuller, richer life.

    Working under the leadership of the Service and Support Administration Manager and direct supervision of the Family and Children First Council Manager, the Community Health Worker (CHW) is responsible for helping clients and their families navigate and access community services and adopt healthy behaviors. Through home visits, the CHW assists clients in overcoming barriers to health, social services, education, and employment. The CHW also provides social support, informal counseling, and advocates for individual and community health needs. The CHW expertly navigates the health and human services systems to bridge the gap between communities and health and social service systems.

    ESSENTIAL DUTIES:

    • Interviews clients with problems such as personal and family adjustments, health, finances, employment, food, clothing, housing, utilities, and physical or mental impairments; completes appropriate checklist and enters information from home visits into database no later than 48-hours after client contact.
    • Follows-up with clients via phone calls and visits to the home or other location convenient to the client.
    • Provides referrals for services to community agencies as appropriate, helps clients connect with transportation resources and provides appointment reminders.
    • Uses the Pathways protocols for care coordination activities in managing assigned caseload of clients.
    • Establishes trusting relationships with clients and their families while providing general support and encouragement.
    • Advocates for clients and acts as a liaison between the client/family and community service agencies (i.e., schools, hospitals, support groups).
    • Counsels clients individually, with family or in other small groups regarding plans for meeting needs; aids client to mobilize inner capacities and environmental resources to improve social functioning.
    • Helps clients and their family members through individual or group conferences to understand, accept, and follow medical recommendations.
    • Determines client's eligibility for services such as financial assistance, insurance and other programs in place to assist individuals; refers clients to community resources and other agencies to meet identified needs.
    • Provides and maintains documentation and paperwork in compliance with CHW requirements, Tuscarawas County Board of Developmental Disabilities’ policies and procedures, HIPAA requirements, DODD and all other related rules and regulations.
    • Demonstrates the ability to function effectively with management staff and as a member of a team with other workers at the worksite and other settings.
    • Demonstrates written and verbal communication skills; establishes and maintains a working rapport with families, other service providers, staff of cooperating agencies, and the general public.
    • Maintains composure and poise with emotional individuals, families, caregivers, visitors, and others.
    • Participates in meetings, trainings, assigned committees and in-services for TuscBDD and CHW/Bridges to Wellness as requested.
    • Demonstrates regular and predictable attendance.
    • Follows Board policies and procedures, applicable laws, rules, regulations and standards.
    • Carries out related duties as requested.
