Recommends changes and/or additions to programs that reflect the changing needs of employees. Being a Wellness Program Administrator creates employee communications that encourage participation in wellness activities and health screening programs. May require a bachelor's degree in area of specialty. Additionally, Wellness Program Administrator typically reports to a supervisor. The Wellness Program Administrator occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function.     Summary of Job Description:

    This position will help create awareness, motivation, and the tools necessary to help individuals at the program develop and maintain a well-rounded, healthy lifestyle. The wellness consultant will complete an initial assessment of each members’ health and will monitor their weight and general measurements through ongoing assessments. Wellness Specialist will collaborate with the members and treatment team to create an individualized Wellness goal tied in with their treatment plan. The Wellness Specialist will provide tools and resources to support goal attainment. This person will also be responsible for facilitating group activities and individual sessions.

     

    Specific Responsibilities:

    • Provides individualized health coaching to members to support obtaining their health objectives. 
    • Teaches others about managing their mental and physical health. 
    • Supports members in learning how to make good, healthy choices in terms of health and wellness. 
    • Creates and adapts wellness needs to overcome barriers to good mental and physical health based on needs of, and with input from, members. 
    • Facilitates groups to support improved health outcomes for Members 
    • Works closely with the treatment team to engage members in appropriate services and resources. 
    • Assist members and the team with coordination of care. 
    • Promotes engagement with primary care and health resources in the community. 
    • Completes all documentation on day services are provided. 
    • Completes all tracking and reporting requirements for outcomes and evaluation. 
    • Supports with the development of healthy Menu and meal planning. 

     

    1. Meet with members at least once per month for individual sessions and document all encounters in the Electronic Health Record (EHR) before the end of shift.  
    2. Complete Wellness Assessments as scheduled.
    3. Attend individual supervision sessions with direct supervisor as scheduled.  
    4. Other duties as assigned.

     

    Additional Performance Expectations:

    • Participate in multidisciplinary treatment team and will support and implement interventions and directives as directed by the Team.
    • Always demonstrate compassion and concern when supporting a Member through embracing Project Transition’s Mission and Core Values.
    • Approach Member engagement from a non-judgmental stance understanding that a Member’s behavior is driven by experience, which may include trauma.
    • Treat and speak to Members with supportive kindness even when a Member demonstrates intense behavioral or emotional actions. Staff will show Members dignity and respect for their values and lifestyles.
    • Seek out appropriate support, consultation with Clinician or Psychiatrist, in conjunction with the Program Director/ Manager or obtain supervision, when they are uncertain about how to respond or support a Member effectively.
    • Report back to the Treatment Team any observations of Member behavior that suggests Member may need additional treatment interventions and/or support.
    • Engage with all external parties/ individuals with professionalism and with a positive customer service approach, understanding that they are always representing the organization.

    Qualifications:

    • Minimum of a High School Diploma and/or associate degree with Wellness Certification. Bachelor’s degree in health-related field preferred.
    • Ability to have fun in the workplace. 
    • Current PA Driver’s License and maintain clean driving record. 
    • These services may be provided by a contracted service provider in the community if the provider is able to meet the requirements of the position.
    • Two (2) years of experience in a drug and alcohol setting.
    • Skilled in Microsoft Office
    • High energy individual with strong work ethic and ability to multi-task
    • Must be a self-motivator who is able to work in a self-directed manner. 
    • Ability to maintain confidentiality.
