Wellness Program Manager designs and implements wellness and work-life programs and policies to build a positive and appealing company culture that will enhance employee engagement, facilitate recruiting, and support productivity improvements. Partners with insurers, healthcare providers, and vendors to introduce wellbeing awareness with on-site health events, classes, assessments/screenings, and information resources. Being a Wellness Program Manager develops and implements work-life programs to provide employees with flexible work arrangements, financial planning, family/child care resources, transportation, memberships, discounts, and educational opportunities. Collects and analyzes information about employment trends and employee workplace needs by conducting surveys or feedback sessions. Description of Duties:
(Houston, Peach, & Crawford County, GA)
The role of the High-Risk Monitoring Case Manager is to utilize evidence-based suicide prevention strategies and practices for Middle Flint virtual clients who are identified as high risk for suicide. While access to ongoing care is sought, HRM Case Manager will monitor for suicide risk within forty-eight (48) hours after the safety plan has been developed through direct face to face contact, and then weekly until the individual is safely established in care. The HRM Case Manager will monitor each individual by telephone, in person, or using technology (i.e. email or text), checking for consumer safety at each contact to assess level of suicide risk and imminent danger. Monitoring for suicide will involve use of evidence-based tools, to include the Columbia Suicide Severity Rating Scale (C-SSRS), the Safety Plan or other evidence-based tools. The HRM Case Manager will also coordinate care to ensure that clients have appropriate follow-up to outpatient appointments as well as ensure that there are appropriate resources in place so that the individuals' established individual recovery plan (IRP) is followed. Participates in Treatment Team Meetings and collaborates with various interagency departments and/or other community resources as needed in order to help with stabilizing the individual. The HRM Case Manager may be required to complete Housing surveys for those individuals that are having difficulty with stable residency which may be causing mental distress. This Case Manager will be required to complete no less then (2) contacts with individuals per month, with at least (1) contact being face to face and (1) contact as collateral. This should occur for no less than 4 months with ongoing services as determined by the needs of the client. All documentation should be completed according to the agency policy. HRM Case Manager will be responsible for meeting the productivity standards of no less than 55% or higher each month. Transporting individuals served is a requirement.
Minimum Qualifications: Bachelor’s Degree in psychology or related field. Must have a valid, unrestricted Georgia Driver’s License. Must be computer literate.
Preferred Qualifications: Three (3) to five (5) years of experience working with individuals with Behavioral AND/OR Substance Abuse Disorders.
