Wellness Program Manager jobs in Houston, TX

Mental health case managers evaluate patients undergoing serious mental health conditions and connect them with the services and resources needed to meet their recovery goals. They may offer patients access to various treatment methods, from rehabilitation to recreational services. To determine the most effective treatments to provide patients, case managers conduct thorough assessments, including mental well-being and physical exams. From there, mental health case managers work closely with patients to build personalized recovery plans that summarize the goals and strategies developed for the patient.

Other key responsibilities of mental health case managers include:

*Evaluating the goals and needs of patients to develop targeted treatment plans or support groups for those undergoing mental health conditions like eating disorders, substance abuse and mental illness *Working one-on-one with patients to help them develop coping mechanisms and set and achieve long-term goals for their mental well-being *Conducting in-depth assessments of patients' mental health states to create informed and effective treatment plans *Adding notes to patients' files and records based on evaluations and treatment progress *Creating personalized treatment plans designed to overcome their mental health and life goals *Remaining calm and rational when resolving certain crisis patient situations that can occur unexpectedly during treatments or examinations *Building after-care treatment plans and communicating them to patients for use once they're released from treatments *Performing home visits on released patients to evaluate their progress and ensure they're functioning on their own *Helping clients with their day-to-day challenges *Collaborating with other health care specialists to provide effective treatment and care to patients *Counseling patients and their families *Acting as patient advocate
