Summary: North Central Family Medical Center is a non-profit Federally Qualified Health Center (FQHC) providing affordable, accessible healthcare and pharmacy services to patients of all ages with locations in Rock Hill and Chester.
Responsible for performing and/or enabling the efficient flow of patients and assisting in the provision of quality health services. Performs health care acts that require knowledge, judgment and skill and are performed under the supervision of the physician or midlevel healthcare provider.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Leadership
- Leads Morning Huddle with department members using provided forms
- Scrub the provider schedule for mistakes in scheduling in preparation for next clinic day
- Reviews provider schedule/procedures with department members
- Reviews supply needs with department members
- Updates department members of any announcements
- Schedule staggered lunches for department members
Women’s Health Floor
- Greet patients
- Monitor patient flow
- Escort patients to appropriate areas (room, lab, cashier desk, etc.)
- Prepare patients to be seen by the provider
- Perform routine health care data collection
- Uses EMR appropriately including messaging, scheduling appointments, review of labs/notes and history documentation
- Document all necessary patient data needed for patient care
- Assist medical provider with procedures, as needed
- Implement nursing interventions and tasks
- Provide basic teaching for health promotion and maintenance
- Performs Prenatal Screening visits
- Receives Obstetric and Women’s Health triage calls
- Cleans and maintains equipment
- Perform procedures as ordered by the physician
- Reschedule patients when provider is out of office
- Collect mail from mail room for providers twice daily
Organize mail per provider preference
- Complete NextGen (Electronic Medical Records) tasks for Obstetric provider within 48 hours
- Administer medications/immunizations as ordered by providers
- Refill medication per NCFMC policy and Scope of Practice
Centering Pregnancy
- Responsible for maintaining the Centering Pregnancy program at North Central
- Participant in Centering Health Institute (CHI) Facilitator Training
- Oversee the logistics for implementation of the Centering Pregnancy Cohorts
- Oversee basic evaluation of the groups, collecting and collating patient evaluations
- Responsible for scheduling CP Guest Speakers (Lactation and Pediatrics)
- Work with the Centering Health Institute (CHI) site consultant on challenges
- Maintenance of patient supplies, patient notebooks and healthy snacks
- Responds to patient inquiries and concerns about CP Group via phone calls or online patient portal messages
- Participates in Patient Care Quality improvements projects
- Supporting CP staff, sends patients welcome letter, general information, and postpartum reunion reminder
- Schedules patients for GAP visits in traditional clinic for any missed Centering sessions
- Updates promotional material (posters, flyers, website content, PAMF social media, etc.) for all public access to Centering information
- Ensures ideal group size, monitors and maintains group census
- Drafts and delivers Bi-Monthly Centering Update Newsletter to department
Rapid Response
- Respond to Rapid Responses on assigned suite days
- Inspect Rapid Response (RR) bag each assigned week for supplies and needs, notify Nurse Manager of needs for RR bag
- Complete Incident Report online
- Maintains confidentiality of all patient information.
Other Duties as Assigned
- During times of staff shortage, must be willing to fill in as needed (including other departments and travel to other sites)
- Read and document TB Tests
- Cover the Women’s Health floor in during times of short staffing
Qualifications:

Education/Experience:
- Graduate of an approved school/training program for registered nurse proof of licensure
- Basic Life Support certification
- A minimum of 2 years ob/gyn nursing experience
