The role of Director of Social Services will consist of assisting in meeting the mental, psychological, and psychosocial needs of our residents. This is a growing, fast-paced facility, so prior experience in social services within a long-term care setting is a must.
Major Duties
· Develop and implement social service policies and procedures to meet residents needs in compliance with federal, state, and local regulations.
· Obtain pertinent social data about personal and family problems medically related to the residents illness.
· Lead care plan meetings
· Act as a discharge planning coordinator for the health care center and each resident
· Completion of Social Services part of MDS using Point Click Care
· Maintain a working resident council, family council, ethics committee and grievance committee
· Prepare the initial Social Service Assessment on each resident including past, present, social and functional history and plan of care for each resident
· Assist and provide with identified referrals as needed (i.e, dental, hearing, vision, counseling, psychiatrist, and any other diagnostic service)
· Make arrangements for obtaining adaptive equipment, clothing, and personal items
