We are seeking a dedicated Social Service Coordinator to join our team. The Service Coordinator will play a crucial role in managing programs, developing relationships, and providing administrative support in the field of behavioral health.

Duties:

- Manage and coordinate care plans for residents
- Develop and maintain relationships with clients, families, and service providers
- Support staff in implementation for behavior modification
- Assist in program development and improvement
- Complete assessments

Qualifications:

- Experience in program management, particularly in the field of behavioral health
- Strong relationship management skills
- Proficiency in program development and administrative tasks
- Knowledge of working with the elderly
- Background in social work or related field (recommended)
