Summary:
The APP will engage in delivering high-level direct patient care in a Post-Acute Care environment, focusing on the elderly and those with chronic illnesses.

Essential Duties and Responsibilities:

• Coordinate with other healthcare disciplines, such as Therapists, Specialists, and Social Workers to ensure comprehensive patient care.
• Address patient condition changes, review ongoing medications, interpret orders and diagnostic tests, and take appropriate action on clinical abnormalities.
• Perform preventive care assessments, including Annual Wellness Visits and possibly Annual History and Physicals based on facility needs.
• Engage with families of patients and participate in conferences to discuss the well-being and treatment plans of residents.
• Work collaboratively with physicians and other medical professionals to manage overall patient care.
• Manage interventions and provide direct episodic care, including necessary re-evaluations.
• Undertake additional responsibilities as assigned to enhance patient care and outcomes.

Qualifications:

• Must hold a valid Nurse Practitioner (NP) or Physician Assistant-Certified (PA-C) license in Massachusetts.
• Required: A Master's Degree or higher in Nursing from an accredited institution.
• A minimum of 2 years experience in primary care nursing is essential.
• Critical thinking abilities, particularly in clinical decision-making and care coordination relevant to elderly and chronically ill populations.
• Exceptional leadership, communication, interpersonal, and problem-solving skills.
• Must be highly organized, punctual, and capable of working efficiently within a collaborative environment.

Vaccination Policy:
Compliance with all applicable guidelines regarding vaccination is mandatory for all employees starting on their first day of employment.

Work Schedule:
This role requires a minimum of 4 hours per day, accommodating patient care within operational hours from 8:00 AM to 6:00 PM. On-call duties are expected 3-4 times per month during nights and/or weekends. Administrative tasks and chart work may be completed remotely.
