This position is located at the Colorado Mental Health Institute at Fort Logan (CMHIFL). CMHIFL is a 94 bed psychiatric facility located in Denver that is dedicated to meeting the needs of the community by providing mental health treatment to adult clients in the metro area. The Colorado Mental Health Institute at Fort Logan (CMHI-FL) is one of two state hospitals serving our community. The vision of CMHIFL is to provide the highest quality mental health services to persons with complex, serious, and persistent mental illness within the resources available. Our mission is to support those individuals in achieving their recovery goals and reaching their full potential. CMHIFL provides inpatient treatment to adult patients, generally between the ages of 18-59. The hospital currently has 138 adult inpatient beds, over six treatment milieus serving civil and forensic individuals. The Social Worker III provides supervision to the social work interns training program and supports execution of the hospital’s mission, vision, and values within the social work department and in collaboration with leadership. As a student supervisor, the Social Worker III is responsible for clinical supervision The Social Worker III provides autonomous clinical service provision by ensuring their work product contributions are reviewed, complete, and timely. This includes treatment plans, documentation, assessments, and therapeutic interventions. This position is also responsible for contributing to the implementation of various departmental and discipline specific projects and supports implementation within the team.
The Social Worker III is responsible for individual therapy, group therapy, and family therapy. Individual and family therapy are provided as needed, which is determined in the treatment plan meetings and in collaboration with the treatment team. Group therapy is offered to all patients and group assignment shall be determined by clinical and individualized need. The Social Worker III is responsible for developing group materials appropriate for patient specific needs and leading group therapy sessions. When providing such treatment, the position relies upon social work theories and concepts relevant to the needs of the patient and their support systems in an effort to support symptom reduction. Interventions are individualized and tailored to issues related to age, ethnicity, culture, religion, gender identity, and sexuality. The social work team communicates treatment needs, barriers, expectations, and changes in behavior to the individual's support system throughout their admission cycle. This position also provides psychoeducation to an individual's support system and community stakeholders in an effort to solicit understanding and participation in the treatment process. This position monitors the effectiveness of various treatment interventions tailored to reach specific treatment goals and objectives by leading the treatment planning meetings for all patients and ensuring goals are specific, measurable, attainable, relevant, and time-based (SMART) and in accordance with regulatory and licensing requirements, as well as best practices.
The Social Worker III is responsible for their own treatment caseload and is required to ensure all coordination of resources is complete in a timely manner. This may include ensuring submission of benefits, identification of appropriate benefits, guardianship needs, Momentum referrals, obtaining identification, securing a payee, applying for Medicaid/Medicare, and submitting referrals to potential discharge locations. The social worker must work closely with the Benefits Specialist and Reintegration Specialist and assist in completion and monitoring of such documents and referrals and may collaborate with legally appointed guardians or members of the patient’s support system to obtain the necessary information and navigate the process. This social worker identifies appropriate discharge locations in collaboration with the treatment team and liaisons with those placements to support continuity of care. In addition, this position and their team coordinate day passes off-campus, attend placement visits, coordinate family and other visits, and attend other off-campus appointments as needed. The Social Worker III often attends interagency staff meetings and is responsible for providing education to stakeholders regarding the Social Work Department and building relationships across various agencies. This position collaborates with their supervisor to ensure to accurately and efficiently navigate various systemic barriers and timely and accurate processes are completed. All efforts are geared toward supporting discharge into the least restrictive environment.
Minimum Qualifications:
Current, valid licensure as a Social Worker (LCSW) from the Colorado State Board of Social Work Examiners.

One year of experience as a licensed clinical social worker.
