Leisure Chateau Care Center is looking to hire an experienced Director of Social Work.

Qualifications

• Bachelor’s degree required. Master’s degree strongly preferred.
• LSW licensure required; LCSW or LICSW strongly preferred.
• Experience as a social worker within a nursing home or rehabilitation center.
• Must be a supportive team member, contribute to and be an example of team work and team concept.
• Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
• Communication skills with the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
• Responsibilities
• Interview resident/families as necessary and in a private setting to obtain social history.
• Develop preliminary and comprehensive assessments of the social service needs of each resident.
• Ensure that social services components of the MDS are completed and signed on a timely basis.
• Involve the resident/family in planning social service programs when possible in a private setting.
• Refer resident/families to appropriate social service agencies when needed
• Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident.
• Develop and implement policies and procedures for the identification of medically related social and emotional needs of the resident.
• Participate in community planning related to the interests of residents
• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.
