Bereavement Coordinator plans and provides bereavement counseling and support to survivors. Assesses individual needs and coordinates with multi-disciplinary teams to develop and deliver a holistic support plan. Being a Bereavement Coordinator guides survivors to appropriate bereavement resources. Facilitates grief support group sessions. Additionally, Bereavement Coordinator requires a bachelor's degree in social work. May require the applicable State License to Practice Clinical Social Work. Typically reports to a manager. The Bereavement Coordinator occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function.     Job Summary:

    Coordinates and organizes the grief support program and services provided by the center in Home Health. Acts as the main coordinator of the volunteers, medical staff and others who are working with the bereaved. Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group. Coordinates small program(s) with limited budget/impact.

    Job Requirements:

    Education and Work Experience:

    • High School Education/GED or equivalent: Preferred
    • Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred
    • Five years' relevant experience: Preferred

    Essential Functions:

    • Assesses needs, offers counseling, provides guidance and explains the services of the bereavement program to the patient(s) and/or families of the bereaved.
    • Organizes and coordinates the services provided by the program, such as arranging counseling, scheduling support groups, providing information on funeral and memorial services, and other bereavement services. Additional job duties include creating bereavement materials, documenting bereavement services given to clients.
    • Collaborates and communicates with the bereavement team members, families, service providers, and community at large.
    • Performs other job-related duties as assigned.
