The four core areas of targeted case management are assessment, development of service plans, referring activities, and monitoring of services.

A Day in the Life of Targeted Case Manager:

• Advocate, support, and guide youth and families in obtaining the needed services including, but not limited to, advocacy, coordination of the various services, education regarding community resources, and referral of a youth/family to available services within the community.
• Maintain contact with other service providers to ensure the unique needs of the youth and family are best met.
• Research the various services offered within the community to identify services that would best meet the unique and individual needs of youth and families.
• Work with the youth, the youth's family, and other professionals involved to develop a treatment/service plan that addresses the specific needs of the youth including, facilitating, and leading the treatment team meetings.
• Participate and engage in monthly supervision/coaching meetings with the supervisor.

What You Need to Qualify:

• A Bachelor’s degree in a mental health-related field, human services, social work, psychology, or related field of study is required.
  • An equivalent combination of education and experience may be considered in lieu of a degree.
• 21 years of age or older
• Driver’s License to operate a passenger vehicle.
• Display regular, predictable, and reliable attendance at work
• Willingness and ability to obtain certification in First Aid/CPR and Violence Intervention Prevention.
• Successfully complete Child and Adolescent Service Intensity Instrument (CASII) training within 30 days of hire.
• Knowledge of the specific treatment/mental health needs of youth with emotional and psychiatric problems and their families
• Excellent written and verbal communication skills
• Knowledge of local community resources for youth and families.
• Good computer/technology skills. Able to quickly learn and use YBGR selected software solutions.
• Criminal / Child Abuse Clearance: Must pass criminal record and child abuse registry background check as required by the State of Montana.

Preferred Qualifications

• 2 (two) years of professional experience in case or treatment management is preferred.
• 2 (two) years of professional experience working with youth with serious emotional disturbances is preferred.
