A Case Manager to service Long Term Care Cases in the Aiken & Barnwell County Areas for the mentally and/or physically disabled individuals in the community ages 18 and up. CLTC experienced case managers are encouraged to apply. This is a Home Community Based Service Position which requires traveling to individual's homes once every 3 months and once every 365 days to complete a Qualifying Reevaluation to reassess abilities.
Education/Professional Requirements: You Must Have …
* A Bachelor's or Master's degree in a health and/or human services field with a minimum of 2 years’ experience in each of assessments, service and/or care planning and case management with clients. This must be fully documented on your resume. Only 6 months will be used toward the two years minimum of experience in all areas from an internship.
* If a State Professional License is held then the minimum 2 years' experience is waived.

* Provide a Certified Copy of Your Transcript or Degree (Verifiable).
* Pass a Criminal Background Check (No Felonies).
* Pass a Drug Test.
* Have Within the Last 12 Months or Obtain an Updated TB Test.
* Have a Valid and Unexpired Driver's License.
* Ability to Obtain a 10 Year Motor Vehicle Record South Carolina DMV.
* Have a Dependable Vehicle with Proof of Registration and Insurance Coverage.
* Maintain a Smart Cell Phone w/ Active Service/Mobile App Use Clocking-In and Out.
* Have Data & Hot Spot Available on Your Cell Phone for Mobile App/Computer Use.
* Be Independently Proficient in the Use of Computers and its Applicable Software.
* Work Independently with Minimal Management.
* Be Organized, Punctual, Ability to Create Your Own Schedule and Meet All Deadlines.
* Be Available Monday - Friday from 8:30 am - 5:00 pm via Phone.
* Be Available to conduct Home Visits Monday - Friday between 7am - 7pm, No Weekends.
* Be Available to Attend Agency/State Mandatory Training's, Yearly Conferences and Monthly Meetings.
