Case Manager - Home Care/Hospice jobs in North Carolina

A Caring Heart Case Management is Seeking a Group Home Manager for our All Female Group Home. The Group Home Manager is responsible for the day-to-day operation including the overall supervision, scheduling and coordination of all staff. The responsibilities will be for a 3-Bed Group Home.

• Monday - Friday
• 8 hour shift
• On Call Availability required
• Group support settings
• Flexible hours
• Will have to work (fill-in) various shifts
• Additional 8 hours per week for administrative duties

Requirements:

• Must be dependable with a good work ethic.
• Must have a reliable vehicle and safe driving record.
• Must have a valid drivers license and state minimum auto insurance.
• Background checks will be performed on all applicants.
• Willingness/highly motivated to assist clients with working-out and building muscle tone.
• Confidence in working with clients that exhibit aggressive behaviors.
• Self-control and patience when dealing with escalated behaviors i.e. verbal provocation.
• Strong characteristics to handle individuals with explosive disorders.
• Ability to learn and execute de-escalation strategies to handle crisis situations in a competent manner.

Nice-to-have:

• Bi-lingual (English and Spanish) is a plus.

About This Position (Habilitation Technician):

• All trainings will be provided by A Caring Heart Case Management Inc.
• Direct support services provide instruction to individuals to help them learn, retain, or improve skills for maximal functioning.
• Skill building focuses on self-care, household maintenance, social and communication skills, money management, social and community inclusion, mobility employment, and self-regulation.
• Services will be provided in the home, community, and day program.
