Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified.     Position Summary

    Provide quality services to all shelter residents who are victims of domestic violence, street crime, elderly abuse, or homeless women and children. Provides direct, ongoing support and expertise through comprehensive assessment, planning, implementation, linkage to resources and overall evaluation and monitoring of individual and family needs. Promotes continuity of care through integrating the functions of case management, victim services, job development, children services, the medical clinic, life skills training and education, mental health services and pet care services. Maintains accountability of care through case planning, the coordination of referral services participation in multidisciplinary team meetings, and discharge planning of all assigned clients. Works directly with clients in crisis.

    Pre-employment FBI background report, drug screening and NV State CANs check required.

    Essential Duties & Responsibilities

    • Support The Shade Tree in their overall mission of providing safe shelter to homeless and abused women and children in crisis and offering life-changing services promoting stability, dignity and self-reliance.
    • Assess families and individuals for strengths, capabilities and barriers, including family and social history, employment, behavioral health, criminal history and substance use to determine level of functioning and services necessary to meet their needs.
    • Collect supplementary information needed to assist clients, such as employment and education records, identification documents, medical records and school reports.
    • Assist families and individuals with identifying, establishing and meeting goals as well as identifying appropriate steps towards achieving goals, including modifying goals as needed, and evaluation of progress.
    • Address legal issues if needed, such as outstanding warrants and tickets, child abuse/Child Protective Services involvement, assisting with hearings and providing testimony to inform of custody arrangements, and any other legal services outlined in the client’s case plan.
    • Facilitate and coordinate supportive activities with other program staff including: employment assistance activities, such as job readiness and workplace skills training, children’s services and life skills classes.
    • Maintain precise/accurate documentation of case management services, on all assigned clients, through the use of paper-based client files and entries into the Bridges database. Document all client encounters and contact made on behalf of clients.
    • Assist clients in obtaining necessary documents such as birth certificates, state identification and Social Security cards, medical and education records if needed.
    • Identify and provide emergency crisis management services and intervention
    • Perform weekly follow-ups assessing and documenting the quality and effectiveness of services provided.
    • Cultivate caring, nonjudgmental, relationships with individuals and families; treat all clients with dignity and respect.
    • Set and observe appropriate boundaries with clients; observe client confidentiality and HIPPA protocols.
    • Advise and assist clients in obtaining assistance from social service programs such as government programs, non-profit programs and linkage with other community services.
    • Serve as a liaison and conduct service coordination meetings with outside agencies as needed to assist individuals and families.
    • Assist in locating and referring clients to appropriate levels of housing and rental assistance programs.
    • Identify and secure alternative placement of special needs clients who are not deemed appropriate for communal living or who would put themselves, other residents, staff members, or facilities at risk.
    • Assess clients and determine appropriateness for referral to on- and off-site assistance programs such as job development and vocational rehabilitation programs, GED preparation and literacy classes, and English as a Second Language classes.
    • Assist clients with preparation of forms such as housing leases, and service applications
    • Prepare and present Service and Behavior Contracts as necessary.
    • Communicate information to co-workers and other departments as necessary and required to ensure effective continuity of service.
    • Maintain and report applicable statistics regarding programs and client services for grant and other funding reports in a timely manner.
    • Ensure resources are used appropriately by eligible individuals and households; avoid duplication of services being offered by other providers.
    • Enforce agency policies and procedures relating to client responsibilities.
    • Attend all mandatory staff meetings and training sessions.
    • React to changes productively and handle other tasks as they arise or are assigned.
    • Communicate and interact with co-workers, clients, volunteers, and visitors effectively and professionally.
    • Represent and promote The Shade Tree and Noah’s Animal House services and programs throughout the community.
    • Assist in transportation for clients to events.
    • Must have a valid Driver’s License.

    Education, Training and Experience Requirements

    • Employment is contingent upon (1) a satisfactory FBI background report, (2) the passage of a pre-employment drug screening, and (3) a clear Nevada Child Abuse and Neglect Screening (CANS).
    • High school diploma required. Bachelor’s degree in Public Administration, Sociology, Social Work or related field preferred. Must have at least two years in Human Services or Customer Service.
    • One year of related social service experience.
    • Experience with MS Office: Word, Excel, Outlook, and internet research.
    • Experience in facilitating trainings, outreach, and presentations.
    • Experience in effectively working with people of varied socio-economic and cultural backgrounds and disabilities.
    • Outstanding organizational, verbal and written communication skills
    • Perform daily duties to achieve excepted outcomes and performance measures with minimal supervision.
    • CPR / AED Certification.
