Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. This challenging and rewarding position is responsible for supporting adults with disabling mental illness. Case Managers link and refer clients to community based social services; provide advocacy for their clients at all times; and support clients through the development of an individualized treatment plan in increasing their quality of life. 3 Rivers Mental Health Center is committed to a developmental path different model of conceptualizing adult mental illness. Requirements: Applicants must have the knowledge and skills needed to effectively perform case management duties.
Minimum qualifications for a Case Manager are:
1. Preferred bachelor's degree in a human services field with cumulatively one year of experience working with individuals of any age with mental illnesses or;
2. Applicants without a Bachelors degree who, as providers, consumers, or advocates of mental health services have gained significant experience and developed the necessary skills, may also be considered for employment as intensive case managers. The successful applicant must possess a valid drivers license and a licensed and insured vehicle.
Compensation:
The salary described in this post is a starting salary based on experience at time of hire.
