Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Looking to add a Case Manager to our amazing team! Looking for 6 or more years of experience with IDD or a licensed social worker and/or a bachelor's degree in social work, Psychology, Counseling, Nursing, Special Education or a closely related field. You will be working with Individuals with Developmental Disabilities. You will make phone calls, meetings and have monthly visits to address any concerns the Individuals may have. You will be responsible for following up on appointments and any issues that may arise. A Case Manager is required to ensure the individual is receiving the requested services and each of their needs are being met. You will be required to ensure the individuals health and safety.
