Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Job Summary
Therapeutic Intervention Services Inc. is a provider of behavioral health care and support services in the state of South Carolina. We deliver community-based treatment, support and services for persons with severe mental illnesses, children with severe emotional and behavioral disorders, and individuals with developmental or intellectual disabilities. We offer community-based services for adults and adolescents with addictions and/or behavioral difficulty.
Therapeutic Intervention Services Inc. is currently seeking Qualified Mental Health Professionals who have experience providing mental health services to individuals with developmental or intellectual disabilities.
Qualifications: (Must possess one of the following along with at least one year of behavioral health experience along with a valid driver’s license)
-master's degree in psychology from an accredited college or university with at least one year of clinical experience
- a bachelor's degree in human services or related field (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling) from an accredited college and with at least one year of clinical experience providing direct services to individuals with a diagnosis of mental illness
Duties will include but not limited to:
-Provide direct support in the home and/or community
-Develop and implement individualized service plans with documentation progress and quarterly reports-Training individuals on daily living skills
-Case coordination with other providers of care (primary care physician, therapist etc.)
-Linkage to community based resources as needed
