RN Case Manager Needed!!! Pay Per Visit - flexible working schedule.

Care in the Home, a busy Home Health Agency in Wilmette, Illinois is seeking a per visit RN to make visits in our service area. This would include visits as far north as Zion and south to Lake Forest. We offer a very competitive per visit rate. The responsibilities of the job are below.

The staff Registered Nurse Case Manager has clinical nursing knowledge, physical assessment, teaching and procedure skills and ability to be directly responsible for assessing, planning, providing and documenting nursing care for designated patients in accordance with the physician's plan of care and Agency policies and procedures. The provision of quality care is accomplished by implementing, supervising and coordinating services provided and utilizing Agency and community resources to meet patients’ needs. The staff Registered Nurse provides supervision to LPNs and Aides and completes documentation accurately and timely in the electronic record. The Registered Nurse performs duties within the scope of the State’s Nurse Practice Act and in compliance with state, federal, Medicare, HIPAA, and other applicable regulations and applicable standards.

A minimum of 1 year previous home health experience is required as well as a general knowledge of charting electronically (EMR).
